If your insurance provider is out-of-network I ask that you pay my fee up front and then I will give you a statement at the end of every month for you to send into your insurance company to get reimbursed.
Check your out of network benefits and deductible.
Whether I am in-network or out-of-network these are things that you need to ask your insurance provider:
What are the mental health benefits?
What services does my plan cover (i.e. individual counseling only, couples counseling, family counseling, etc)
What is my deductible and how much of it has been met?
Is there a limit to the number of sessions per calendar year your plan covers?
What is your responsibility for your sessions?
Do you have a co-pay? A fee that is paid at the time of service every session (common co-pays are $20, $25, $30 or $35)
Do you have a co-insurance? co-insurance is a percentage that you are responsible for after the insurance pays the provider. So for example, with some plans insurance companies pay 90% of the fee to the provider and you may be responsible for 10% of the provider’s fee).
Is pre- authorization required?
